To Register your child for Soccer:
1. Print and fill out EL Rec. Dept. Registration Form
2. Print and fill out EL Medical Form
3. Pioneer Valley Players ONLY:
Print and fill out MYSA Membership Form
4. Pioneer Valley players ONLY:
Place a 1" x 1" picture of the player (Head shot only) with the players name and age group on the back (e.g. Under 13 girls) in envelope
5. Place appropriate fee(s) in envelope with forms from above with the players name and age group on the back and mail to:

Attn: Soccer
Spring 2010 fees: $70 per player, $90 if the player did NOT play in the Fall 2009 (This cover the cost of the uniform).
